    How To Get A Number And Card

    To get an original or replacement number and card, you'll need to complete an Application for a Social Security Card (Form SS-5), and show documents that prove your age, identity, U.S. citizenship or lawful alien status. The Application for a Social Security Card (SS-5) can be used by anyone who has never been issued a card, needs a replacement card or has changed his or her name.

    To get an application: Complete the SS-5 form.
    • Use the instructions on page 1 of the form.
    • See pages 2 and 3 of the form for the evidence that you will need to submit with your application.
    Take or mail the SS-5 to the nearest Social Security office.
    • Be sure to take or mail the originals or certified copies of your supporting documents along with the form.
    • They will return your original documents right away.
    • Once they have everything they need, they will send your Social Security Card in about two weeks.
    Contact your local Social Security office or call our toll-free number 1-800-772-1213 before you visit the office. Tell them what documents you have. That way you won't have to make a second trip.

    Remember: there is no charge to get a Social Security card. This service is FREE.
